System UIDs Inconsistent

Don't know if anyone else has came across this situation...but it don't look good. Any ideas what needs to be done, does this mean the SD card or the internal card? The device does not seem to be reading the SD card, but when you enter ES File Explorer you can access the SD card??? Clicking on installed apps sent to the SD card through Link2SD states that the apps are not installed. I have done a hard reset with no effect. (Picture included)

System UIDs Inconsistent
UIDs on the system are inconsistent,
you need to wipe your data partition
or your device will be unstable.
